#32: Death of Stuntman, L.A. Heat
In order to honor Dallas’s memory and not speak ill of the sea, I will go ahead and establish that this was not a design or manufacturing error. There is no question that Dallas’s design and production were flawless. Rather, it seems the mistake was made at the execution level.

And what a horrendous mistake it was. Anyone could tell you that relying on the precise position of an airbag to survive a three-story fall isn’t exactly what one would call foolproof. So, I think stupidity and carelessness are to blame; complete and utter stupidity.
